RADIUS: Do not include Acct-Terminate-Cause in Accounting-On/Off

Per RFC 2866, 5.10, it is invalid to send Acct-Terminate-Cause in
Accounting-On and Accounting-Off (this is included only when
Acct-Status-Type is set to Stop).
